Transition Planning Timeline
It’s never too early to being the transition planning process. The timeline below is a useful tool that can keep caregivers informed about the steps required for each age group.
Beginning at Age 12
1) Complete age 12 Assessment (Level 1).
2) Apply for OPWDD Eligibility.
3) Attend OPWDD Front Door Orientation.
4) Develop an annual Career Plan.
5) Explore community-based leisure or work-based learning opportunities.
6) SSI/SSDI Medicaid Programs
Ages 15-17
1) Complete an annual Career/Vocational Assessment.
2) Obtain Parents Notification: IEP incorporates transition services. 3) Explore transition services and attend community transition fairs. 4) Obtain work-related documents: SS card, birth certificate, work papers, personal ID (MUST DO BEFORE AGE 21). 5) Get parental consent required for agency representation (MUST DO BEFORE AGE 21). 6) Begin identifying career interests and post-secondary goals. 7) Begin to self-advocate. Attend and participate in IEP meetings. 8) Develop the initial transition plan at the annual IEP meeting. 9) Explore volunteer or employment opportunities. 10) Explore housing or residential opportunities. 11) Investigate supportive community agencies or organizations. 12) Practice travel, transportation/mobility strategies. Use public transportation. 13) Understand guardianship, age of majority issues, and alternatives (BEFORE 18). 14) Explore benefits of a Supplemental Needs Trust.
Age 18 and Beyond
1) Look into Adult Career, continuing education, Vocational Rehabilitation (ACCES-VR). 2) Complete IEP transition components; annual review and update. 3) Explore volunteer or employment opportunities and transportation to work. 4) Explore housing or residential opportunities and living arrangements (independent, roommate, group living, etc.). 5) Explore supportive community agencies or organizations. 6) Tour adult service agencies. 7) Create transportation/mobility strategies - Explore assistive technology options. 8) Begin financial planning - understand how savings can impact benefits. 9) Explore opening a NY Able Account. 10) Discuss post school options like education, career, technical training, and college. 11) Apply for college (MUST DO BEFORE 21) 12) Repply for SSI, as needed (MUST DO BEFORE 21) 13) Inform insurance company of disability status. 14) Explore adult healthcare options and health insurance rider to continue eligibility. 15) Apply for driver's license, non-driver's ID, and para-transit. 16) ALL males register for Selective Service. 17) Register to Vote.
