Rehabilitation Counselor

This position works under the supervision of the site Manager to provide direct services to individuals in residential and community settings. The Rehabilitation Counselor is responsible for creating a normalized, therapeutic, and safe environment while promoting residents’ positive self-esteem through supportive interactions and respect for individual rights. Day Habilitation Assistant Manager

Seeking experienced and enthusiastic staff who want to make a difference in the lives of our individuals with developmental disabilities.

The Day Habilitation Assistant Manager is responsible for the oversight of staff and scheduling interesting and innovative habilitative programming for the people we support in the certified site, and seeking out new volunteer sites. The Day Habilitation Assistant Mgr. works closely with the Day Habilitation Manager to facilitate community inclusion, integration, and relationship building according to the individual’s Life Plan.

The Day Habilitation Assistant Manager also works closely with Day Habilitation Manager and the Director of Day Programs in the implementation and management of the daily routine. This position requires the Day Habilitation Assistant Manager to assume coverage in the Group Day Hab in the program is in need of additional staffing.

Minimally 2 years’ experience in a With or Without Walls Day Program or equivalent. Supervisory experience required.

Community Habilitation Specialist

Support people in recreational, social, and skill-building activities within their homes and the community to achieve their goals. Encourage independence and personal choice. Work closely with team, family and community members while developing your knowledge and skillset through paid training. 

Food Service Worker

Community Based Services is actively seeking one full time Food Service Worker to provide support for our agency. The Food Service Worker will assist the Director of Food Services in the preparation of supply distribution for the residences, as well as any events needed by the agency. The Food Services Worker will also provide assistance in the production of dog treats for our sister company, Good Reasons.

Requirements

  • Must be able to process and store deliveries in an organized manner in accordance to commissary procedure.
  • Must be able to process and package groceries and other supplies as dictated by the weekly inventory sheets in a time sensitive manner.
  • Must be able to distribute and load groceries and other supplies for each residence in a time sensitive manner.
  • Must be able to lift 50 pounds.
  • Must be able to provide direction and assistance in all aspects of the Dog Treat Production from preparing recipes to working with the various pieces of equipment, the packaging process and general cleanup of the Production Facility.
  • Must be able to work effectively in a team environment and possess strong communication skills and attention to detail.
  • Occasionally may need to adjust schedule based on the facility’s needs.

Responsibilities

  • Process, weigh and package groceries and other supplies according to weekly inventory sheets.
  • Load up House Vans when they arrive for their supplies every Thursday.
  • Cleaning of the workplace. This includes breaking down and recycling cardboard, ware-washing (cleaning of pans, utensils, pots, etc.), sanitizing table and work surfaces, sweeping/vacuuming floors and cleaning equipment.
  • Prepare for baking dog treats by scaling out and storing ingredients.
  • Baking dog treats by using Hobart Mixer and Convection Oven.
  • Storing dog treats. They are stored in 50 lb. increments in our storage bins, labeled and refrigerated.
  • Provide encouragement and direction to individuals with developmental disabilities while they perform their job duties.

Administrative Assistant - Nursing Dept

The Nursing Support Coordinator provides clerical and administrative support for the Residence Nurse under the direction of the Director of Nursing Services. The Residential Nursing Assistant will also provide direct support to our individuals as needed in the area of medical appointments, medication administration and daily care. 

Responsibilities

  • Assisting the Residence Nurse with maintaining accurate and complete medical records, including nursing forms, pharmacy reviews, lab work, and consultations. Tracking, scheduling, and following up on individuals’ medical appointments.
  • Assisting with monthly weights and required documentation.
  • Ordering medications and medical supplies; completing pharmacy runs as needed.
  • Maintaining a fully stocked medication room and coordinating orders with the Purchasing Department for briefs and Jevity, including securing MD prescriptions.
  • Updating, auditing, and re-sheeting MAR/TARs, narcotics records, and narcotics count sheets.
  • Checking blister packs and reporting discrepancies or errors to the Residence Nurse.
  • Preparing medical consults, monitoring the medication calendar, and communicating medication changes or urgent follow-ups.
  • Coordinating transportation and staffing for medical appointments with the House Supervisor and DSP staff.
  • Filing controlled drug sheets, G-tube sheets, completed MAR/TARs, and BM sheets after nurse review.
  • Administering medications as needed, including providing medication coverage during short staffing or emergencies (minimum monthly medication administration).
  • Attending medical appointments during short staffing or when requested by the Residence Nurse or House Supervisor.
