Meet the CBS Transition Planning Team

Life’s transitions, even the expected ones, are difficult for all of us. The jump from children’s services to adult services is no different. Students graduating from high school who have IEPs or 504s are met with the extraordinary challenge of navigating new and complex systems. In recent years, CBS has recognized this and we have stepped up to help with a Transition Planning service.
“Transition planning can be overwhelming, and many families don’t know what options are available for their children or what services they will be eligible to receive. The CBS transition team has been established to assist students and caregivers in navigating the systems of care to meet transition goals of graduating students,” explains CBS Transition Coordinator Jennifer Havrilla who has been on the job for a full year, developing relationships with families and school district representatives in Dutchess County.
The CBS Transition Team, which now also includes Transition Coordinator Jasha Woodall, continues to partner with school districts to organize Transition & Resource Fairs to provide support to graduating students and their families. Jasha will be focusing on new partnerships with 13 targeted school districts in Northern Westchester. Whether the student is looking for higher education, employment, day program supports, or a blended schedule that includes multiple services, Jennifer and Jasha are there to help.
This innovative service is not something that is currently provided by other agencies under the OPWDD umbrella. Transition support is something that our team at CBS has identified as a need in the community and has now dedicated resources to providing.
