Community Based Services (CBS) is here to support teens and parents throughout this crucial period. Checking your eligibility!

  1. Start by ensuring your child is approved for OPWDD (Office for People With Developmental Disabilities) Eligibility. This approval is essential for enrolling in transition planning services. 2. Identify services that meet your child’s needs through the NYS eligibility process. A comprehensive list of available services can be found here: Types of Services.
